Home Care Providers in an established, growing and innovative Home Health Agency headquartered in Orange County providing service to clients throughout Southern California.

We are seeking a compassionate Registered Nurse who is looking for exciting growth in the Home Health field in the Orange County area.  



Job purpose


The Assessment/Skilled Nursing Supervisory Field RN is responsible for new and current client assessments and supervision of skilled nurses in the field in accordance with company policies to ensure the highest quality of patient care. The position will be under the direction of the Director of Patient Care Services.


Duties and responsibilities


  • Completes intake information on new clients.
  • Completes re-certification on current clients.
  • Completes assessments and submits documentation to DPCS.
  • Completes home visits on all clients receiving hands-on care, at least bi-monthly.
  • Ensures safe, hand on care provided by nursing staff in the field and completes and submits appropriate supervisory documentation promptly.
  • Reports to DPCS weekly.
  • Works as part of a team with other agency staff to deliver quality client care.
  • Assists in implementing policies and procedures to maintain high-quality professional standards.  
  • Reviews field staff flow sheets/ notes and gives feedback to DPCS.
  • Maintains effective communication within the agency and among staff families and clients. Promotes a positive image of the agency and home care within the community.
  • Adheres to HIPAA and other confidentiality guidelines. 


Work Conditions


Physical Requirements and Work Environment

Performance of duties requires a California Driver's License with travel up to 70% and 30% in the office. The position requires moderate sitting, standing, and walking. The specific vision abilities required by the job include Close vision and the ability to adjust focus between source documents.


Responsibility for Safety in the Workplace

The incumbent is responsible for making sure that any unsafe practices or unsafe areas are brought to the attention of supervision for immediate resolution.




  • Preferred: BSN-RN license with 1-2 years nursing experience.
  • Preferred: RN license with 3 years nursing experience in home health, hospice or home care.
  • Must have a current California Registered Nurse License.
  • Reliable transportation and the ability to travel to clients' homes
  • Experience in private Home Care preferred.
  • Excellent customer service skills
